Your First Visit
Thank you for scheduling your first appointment. Please arrive 15 minutes early to fill out the new patient forms or fill them out prior to your first appointment on the online patient portal. Ensure you have had something to eat at least 1 hour prior to treatment and refrain from caffeine or alcohol consumption. Also, wear loose fitting clothing to make points on the body more easily accessible.
If you are seeking treatment for Cancer support, Advanced Pain Management, or Internal medicine, please bring pertinent medical records regarding prior or current medical treatment, imaging, and most recent lab work.
